Indications/Uses
Symptomatic treatment of OA (arthrosis or degenerative joint disease), RA, juvenile arthritis & ankylosing spondylitis.
Dosage/Direction for Use
Adult OA (arthrosis or degenerative joint disease), RA, ankylosing spondylitis 7.5 mg once daily. May be increased to 15 mg once daily. Max: 15 mg daily. Juvenile arthritis Childn & adolescent 12-18 yr weighing ≥50 kg 15 mg once daily, <50 kg 7.5 mg once daily. Hemodialysis patient Max: 7.5 mg daily.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to meloxicam, aspirin & other NSAIDs. Patients w/ GI & intracranial hemorrhage & bleeding in different parts of the body; GI ulcers or history of GI diseases; dengue hemorrhagic fever; asthma, urticaria or acute nasal cavity inflammation after taking aspirin or NSAIDs. Peri-operative pain treatment in CABG surgery. Severe hepatic & renal impairment. Last trimester pregnancy (≥30 wk) or lactation.
Special Precautions
Discontinue treatment in case of signs & symptoms associated w/ development of hepatic or renal disease, systemic manifestations, persistent or worsening of abnormal liver test; flu-like symptoms eg, fever, vesicle, peeling of skin & mucous membranes including mouth, nose, throat, reproductive organs & conjunctivitis occur. Avoid use in patients w/ dengue hemorrhagic fever or other thrombosis disorders. Increased risk of serious CV thrombotic events, MI & stroke; GI hemorrhage or ulceration; ischemic stroke & heart disease especially w/ prolonged use. Onset of new or worsening existing HTN. Renal papillary necrosis & medullary changes w/ long-term use. Edema. Patients w/ CV disease or risk factors for CV disease; heart failure, liver dysfunction; HTN. Closely monitor BP during initiation & throughout treatment; renal function. Monitor for signs or symptoms of GIT hemorrhage & anemia. Perform CBC & chemistry profile check, including Hb or hematocrit for patients in long-term use. Concomitant use w/ diuretics, ACE inhibitors & AIIA. Renal impairment. Severe hepatic impairment. Avoid use in last trimester of pregnancy (≥30 wk) (premature closure of ductus arteriosus, affecting lung & heart function of fetus). Not recommended in lactation. Childn <2 yr. Higher risk for serious GI ulcer in elderly (particularly in females).
Adverse Reactions
Dyspepsia, nausea, vomiting, abdominal pain, constipation, flatulence, diarrhoea, transitory abnormalities of liver function parameters (eg, raised transaminases or bilirubin), eructation, oesophagitis, gastroduodenal ulcer, occult or macroscopic GI bleeding & perforation, colitis, hepatitis, gastritis; anaemia, blood count disturbances including differential white cell count, leukopenia & thrombocytopenia, cytopenia (in concomitant use w/ MTX); pruritus, skin rash, stomatitis, urticaria, photosensitisation; cough, pharyngitis, upper resp infection, acute asthma; lightheadedness, headache, vertigo, tinnitus, drowsiness, insomnia; oedema, increased BP, palpitations, flush; abnormal renal function parameters (increased serum creatinine &/or urea), acute renal failure; conjunctivitis, visual disturbances including blurred vision; flu-like symptoms. Erythema multiforme, SJS & TEN.
Drug Interactions
Diminished antihypertensive effect of ACE inhibitors & AIIA. Increased plasma conc & adverse effect especially GI ulcers w/ aspirin. Increased plasma clearance w/ bile acid sequestrants eg, cholestyramine. Reduced renal blood flow due to decreased prostaglandin synthesis & reduced natriuresis w/ diuretics eg, furosemide or thiazide. Increased lithium plasma conc. May increase risk of MTX toxicity. Increased risk of GI hemorrhage & other bleeding w/ other NSAIDs & warfarin or antiplatelets or anticoagulants. Risk of toxicity w/ other NSAIDs & cyclosporine.
