Omeprazole
Indications/Uses
Duodenal & gastric ulcer, duodenal ulcer associated w/ H. pylori infection, dyspepsia, erosive oesophagitis, GERD, hypersecretory conditions (eg, Zollinger-Ellison syndrome). Aspiration prophylaxis.
Dosage/Direction for Use
Adult Duodenal ulcer 20 mg daily for 4 wk. Severe case: 40 mg once daily for 4 wk. Duodenal ulcer associated w/ H. pylori infection 20-40 mg + clarithromycin 500 mg & amoxicillin 1 g bid for 10-14 days. Gastric ulcer 40 mg once daily for 8 wk. Dyspepsia 10-20 mg once daily for 2-4 wk. Erosive oesophagitis Initial dose: 20 mg once daily for 4-8 wk. Maintenance dose: 20 mg once daily for max of 12 mth. GERD 20 mg once daily for 4-8 wk. Hypersecretory conditions eg, Zollinger-Ellison syndrome 60 mg once daily. Max: 120 mg daily in divided doses. Duration: Up to 5 yr in some patients. Aspiration prophylaxis 40 mg in evening before surgery, thereafter, 40 mg 2-6 hr prior to operation. Childn Duodenal ulcer associated w/ H. pylori infection 1-2 mg/kg once daily. Max: 40 mg daily or 1-2 mg/kg daily. Erosive oesophagitis & GERD in ≥1 yr weighing ≥ 20 kg 20 mg once daily. Max: 20 mg daily or 3 mg/kg daily. Hepatic impairment 10-20 mg once daily.
Administration
Should be taken on an empty stomach: Take at least 1 hr before meals. Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity to omeprazole or substituted benzimidazole.
Special Precautions
Atrophic gastritis in long-term treatment. May suppress symptoms & diagnosis of gastric malignancy. Different race. Hepatic impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Dizziness, asthenia, headache; abdominal pain, constipation, diarrhea, nausea, vomiting; back & joint pain, cough, rash, URTI, peripheral edema, agitation, insomnia, hemifacial dysesthesia, depression, pancreatitis, thrombocytopenia & loss of taste.
Drug Interactions
Prolonged elimination of diazepam, phenytoin & warfarin. Decreased absorption rate of dasatinib, ketoconazole & itraconazole. Increased plasma conc of voriconazole, clarithromycin & digoxin. Decreased activity of atazanavir. Increased toxicity & adverse effects of cilostazol. Potentiated gastric irritation w/ salicylates. Increased plasma level of sulfonylureas. Delayed absorption & bioavailability w/ sucralfate. May reduce clearance, prolong t½ & increase serum level of benzodiazepines eg, diazepam, triazolam.
