Eletriptan HBr
Indications/Uses
Acute treatment of migraine w/ or w/o aura.
Dosage/Direction for Use
Adult 18-65 yr Initially 40 mg, repeated if necessary after 2 hr. Max: 160 mg/day. Renal impairment Max: 40 mg.
Administration
May be taken with or without food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Severe hepatic impairment. Uncontrolled HTN. CHD including ischaemic heart disease (angina pectoris, previous MI or confirmed silent ischaemia). Coronary artery vasospasm, Prinzmetal's angina. Peripheral vascular disease. History of CVA or transient ischaemic attack. Concomitant use w/ ergotamine, ergotamine derivatives (including methysergide), other 5-HT1 receptor agonists.
Special Precautions
Renal impairment. Concomitant use w/ SNRIs & SSRIs, potent CYP3A4 inhibitors eg, ketoconazole, itraconazole, erythromycin, clarithromycin, josamycin & PIs (ritonavir, indinavir & nelfinavir); other 5-HT1 receptor agonists. Not for management of hemiplegic, ophthalmoplegic or basilar migraine, & treatment of atypical headaches. CV evaluation prior to commencement of therapy. CAD patients (HTN, diabetes, smokers, men >40 yr, post-menopausal women & strong family history of CAD). Patients w/ heart failure & chronic daily headache. Do not use prophylactically. May affect ability to drive or operate machinery. Adolescents 12-17 yr & childn 6-11 yr. Pregnancy & lactation. Elderly >65 yr.
Adverse Reactions
Dizziness, headache, hypertonia, hypoesthesia, myasthenia, paraesthesia, somnolence; vertigo; palpitation, tachycardia; sensation of warmth or flushing; pharyngitis, throat tightness; abdominal pain, dry mouth, dyspepsia, nausea; sweating; back pain, myalgia; asthenia, chest symptoms (pain, tightness, pressure), chills, pain.
Drug Interactions
Increased exposure w/ potent CYP3A4 inhibitors (eg, ketoconazole, itraconazole, erythromycin, clarithromycin, josamycin) & PIs (ritonavir, indinavir & nelfinavir). Increased BP w/ caffeine. Increased risk of serotonin syndrome w/ SSRIs & SNRIs.
