Recombinant human erythropoietin α
Indications/Uses
Anemia associated w/ chronic renal failure in patients w/ or w/o dialysis; HIV-infected patients treated w/ zidovudine; patients w/ non-myeloid malignancies where anemia is due to the effect of concomitantly administered chemotherapy.
Dosage/Direction for Use
IV/SC Anemia associated w/ chronic renal failure Adult 50-100 U/kg 3 times wkly. Ped patients 50 U/kg 3 times wkly. Anemia associated w/ zidovudine-treated HIV-infected patients Adult Initially 100 U/kg 3 times wkly. Ped patients 50-400 U/kg 2-3 times wkly. Anemia associated w/ cancer patients on chemotherapy Adult 150 U/kg 3 times wkly. Ped patients 25-300 U/kg 2-7 times wkly.
Contraindications
Hypersensitivity to mammalian cell-derived products & albumin (human); uncontrolled HTN.
Special Precautions
Discontinue treatment if Ab-mediated pure red cell aplasia develops & test for erythropoietin Abs; anaphylactoid reactions occurs. Determine hematocrit in HIV-infected patients w/ zidovudine-induced anemia or cancer patients w/ chemotherapy-induced anemia at least once a wk, & at least once or twice wkly in patients w/ chronic renal failure anemia. Monitor BP in patients w/ underlying history of HTN or CV disease. Preexisting seizure order. Monitor serum & tissue iron stores during therapy. Acute or chronic inflammation, infection, neoplastic disease or malignancy, underlying myelodysplastic disorders, marrow suppression from uremia, Al overload, hyperparathyroidism/osteitis fibrosa cystica, hypersplenism, hyacute or chronic blood loss, erythrocyte enzyme abnormalities, folic acid or vit B12 deficiency. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
HTN; increased incidence of partial or complete clotting in renal dialysis patients; seizures; pre-dialysis increases in serum K, BUN, creatinine, uric acid & phosphate. Arthralgias; asthenia; diarrhea; edema; headache.
