Should be taken with food: Swallow whole, do not chew/crush.
Contraindications
Manifest heart failure; cardiogenic shock (except for shock induced by arrhythmia); severe bradycardia w/in 3 mth after MI; preexisting high degree SA, AV & intraventricular disorders of impulse conduction; sick sinus syndrome; manifest disorders of electrolyte balance; severe obstructive pulmonary disease; marked hypotension, myasthenia gravis.
Impaired function of liver or kidneys; 1st trimester of pregnancy, lactation; elderly or patients w/ severely impaired myocardial function, dosage must be carefully titrated. May impair ability to drive or operate machinery. May alter both pacing & sensing thresholds of artificial pacemakers.
Occasionally, anorexia, sensation of repletion, nausea, vomiting, bitter taste, numbness of mouth. Sporadically blurred vision; vertigo; allergic skin reactions; circulatory dysregulation in orthostasis. Rarely, fatigue, headache, psychic disorders, extrapyramidal symptoms; bradycardia, SA, AV or intraventricular blocks. Cholestasis. Risk of eliciting ventricular tachycardia or ventricular flutter, fibrillation; diminution of potency & drop in sperm count (high dose); very rarely, fully reversible decrease of white cell, granulocyte & platelet counts; agranulocytosis, rarely observed. Bronchial spasm, convulsion (following overdose).
Drug Interactions
Effect may be potentiated by local anesth, β-blockers, TCAs. May increase plasma level of propranolol, metoprolol, desipramine, cyclosporin, digoxin, theophylline. Plasma conc may rise when taken simultaneously w/ cimetidine or quinidine. May enhance effect of anticoagulant. Simultaneous administration w/ phenobarb or rifamycin may decrease propafenone plasma conc, possibly in sub-therapeutic levels.
