Timolol maleate
Indications/Uses
Elevated IOP in patients w/ ocular HTN or open-angle glaucoma.
Dosage/Direction for Use
1 drop in the affected eye(s) bid. May be reduced to 1 drop once daily if IOP is maintained.
Contraindications
Hypersensitivity. Patients w/ or w/ history of bronchial asthma. Severe COPD, sinus bradycardia, AV block >1st degree, overt cardiac failure, or cardiogenic shock.
Special Precautions
Discontinue at 1st sign or symptom of cardiac failure; in cerebrovascular insufficiency. History of atopy or severe anaphylaxis. Anesth during major surgery; protracted severe hypotension, difficulty maintaining heart rate; choroidal detachment after filtration procedures; mild or moderate COPD; DM; myasthenic conditions. May mask symptoms of acute hypoglycemia; clinical signs of hyperthyroidism eg, tachycardia. Not to be used in angle-closure glaucoma as monotherapy. Increased risk of exacerbation or precipitation; reduced cerebral blood flow. Consider dose reduction prior to surgery. Avoid abrupt w/drawal in suspected thyrotoxicosis. Systemic absorption may occur. Not recommended in concomitant use w/ 2nd topical β-blocker. Avoid concomitant use w/ Ca antagonists in patients w/ impaired cardiac function. Pregnancy & lactation.
Adverse Reactions
Angina pectoris, hypotension; pruritus, rash, urticaria; abdominal pain, diarrhea, indigestion, nausea, vomiting; cramp; confusion, headache; blurred vision, eye burning sensation, dry eyes; depression, hallucinations, psychotic disorder; cough; infectious disease. Cardiac dysrhythmia; MI.
Drug Interactions
Additive effect w/ topical miotics, latanoprost &/or topical or systemic carbonic anhydrase inhibitors; systemic β-adrenergic blockers; reserpine. Prolonged AV conduction w/ β-adrenergic blockers, Ca-channel blockers, cardiac glycosides. Concomitant use w/ oral verapamil. Inhibited metabolism via CYP450 2D6 by quinidine.
