Sumatriptan succinate
Indications/Uses
Acute relief of migraine attacks, w/ or w/o aura.
Dosage/Direction for Use
Adult 50-100 mg. If the patient responded to the 1st dose but the symptoms recur, a 2nd dose may be given, provided that there is a min interval of 2 hr between the 2 doses. No more than 300 mg should be taken in any 24 hr period.
Administration
May be taken with or without food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Previous MI, ischaemic heart disease (IHD), Prinzmetal's angina/coronary vasospasm, peripheral vascular disease or patients who have symptoms or signs consistent w/ IHD. History of cerebrovascular accident (CVA) or transient ischaemic attack (TIA). Severe hepatic impairment. Moderate & severe HTN & mild uncontrolled HTN. Concomitant administration of ergotamine or ergotamine derivatives eg, methysergide, or any triptan/5-hydroxytryptamine1 (5-HT1) receptor agonist; MAOIs. Do not use w/in 2 wk of discontinuation of MAOIs.
Special Precautions
Use only when there is a clear diagnosis of migraine. Not indicated for use in the management of hemiplegic, basilar or ophthalmoplegic migraine. Should exclude other potentially serious neurological conditions (eg, CVA, TIA). Associated w/ transient symptoms eg, chest pain & tightness. Patients w/ risk factors for ischaemic heart disease including heavy smokers or users of nicotine substitution therapies w/o prior CV evaluation; postmenopausal women, males >40 yr; mild controlled HTN; serotonin syndrome eg, altered mental status, autonomic instability & neuromuscular abnormalities; history of seizures or other risk factors which lower the seizure threshold. Impaired hepatic or renal function. Medication overuse headache. Hypersensitivity to sulphonamides. Concomitant use of SSRI/SNRI; triptans & herbal prep containing St. John's wort. Rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption. May impair ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Dizziness, drowsiness, sensory disturbances including paraesthesia & hypoaesthesia; transient increase in BP, flushing; dyspnoea; nausea & vomiting; sensations of heaviness; pain, sensations of heat or cold, pressure or tightness; feelings of weakness, fatigue.
Drug Interactions
Possible increased risk of coronary vasospasm w/ prep containing ergotamine or another 5-HT1 receptor agonist. Interaction may occur w/ MAOIs. Reports of serotonin syndrome w/ SSRIs & SNRIs.
