Fenofibrate (200 M: micronized)
Indications/Uses
Hypercholesterolemia (type IIa) & endogenous hypertriglyceridemia whether isolated (type IV) or associated (type IIb & III) when response to diet alone has been inadequate, when blood cholesterol level remains high in spite of diet &/or when there is a relating risk factor involved.
Dosage/Direction for Use
Adult 100 mg 3 times daily or 300 mg once daily. Cholesterol >4 g/L after diet May be increased to 100 mg 4 times daily. Once normalized, may reduce dose to 100 mg twice daily. 200 M Adult Initially 1 cap daily. Cholesterol level ≥4 g/L after tight diet 1 cap of Hyperchol 100 mg + 1 cap Hyperchol 200 M daily. May be increased to 400 mg (2 cap of Hyperchol 200 M) daily. Maintenance: 1 cap daily. May increase to 300 mg daily in case of rise in blood cholesterol level.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Hepatic or renal dysfunction including biliary disease. Childn.
Special Precautions
Interrupt treatment if SGPT level exceeds 100 IU. Consider a different therapy if after several mth (3-6 mth) of treatment no adequate reduction in serum lipid levels are achieved. Regular monitoring of transaminase levels every 3 mth for >12 mth of initial therapy. Avoid hepatotoxic substances eg, perhexillin maleate & MAOI. Not recommended for pregnant women & nursing mothers.
Adverse Reactions
Mild GI disturbances (dyspeptic type); allergic reactions; myalgia accompanied by increased creatine phosphokinase; modest rise (transient) in serum transaminase levels; anemia, leukopenia, thrombocytopenia; alopaecia; sexual asthenia.
Drug Interactions
Reduce dose of anticoagulants to prevent bleeding complications. In concurrent therapy w/ resin, take the resin at an interval of 2 hr prior to Hyperchol. May produce myopathy in concomitant use of fibrate & HMG-CoA reductase inhibitors. Monitor concurrent use w/ oral hypoglycemic agents w/ other fibrate derivatives.
