Clobazam.
Action
Clobazam is a derivative of benzodiazepines which act as an anxiolytic and anti-convulsive. Increase GABA induction thereby increasing conduction activation in chloride channels.
Indications/Uses
For conditions related to anxiety, tension, sleep disorders caused by mental and emotional disorders.
Dosage/Direction for Use
Adults: 20-30 mg daily. Elderly: 10-15 mg daily. Children 3-15 yr: 5-10 mg daily.
Tablets must be swallowed thoroughly with water, if it is given in a day, the dosage must be divided, a high dose must be taken during the night (before bedtime).
Contraindications
Hypersensitivity; Myasthenia gravis; History of drug or alcohol dependence; Sleep apnea syndrome; Severe respiratory failure or chronic obstructive airway disease, accompanied by a threat of respiratory arrest. Pregnancy & lactation.
Special Precautions
Can cause calls in driving or operating machinery, especially when taken with alcohol; Care should be given to patients with organic disorders in psycho-vegetative and psychosomatic; Patients with drug dependents disorder; Cessation of treatment must be done gradually; Benzodiazepines are given only for a short period of time (2-4), long-term use of clobazam is not recommended; The use of elderly or very weak patients, the dose must be limited to the smallest effective amount; Hypotension; Epilepsy; Amnesia; Paradoxical reaction; Thyroid adenoma; Acute intoxication with CNS depressant drugs; Muscle weakness, ataxia; Glaucoma; Nerve / liver failure and blood dyscrasias; Depression, psychosis and schizophrenia; Drug Dependency.
Adverse Reactions
Fatigue, dry mouth, constipation, loss of appetite, temperature, dizziness, finger tremors. Restlessness, irritability, muscle weakness, difficulty sleeping, symptoms of irritability.
Drug Interactions
Generates mutual potential for sharing drugs that depress CNS, alcohol and anti-convulsions.
If given together with valproic acid, it can increase plasma valproic acid levels.
Together with carbamazepine and phenytoin can increase clobazam metabolism to n-desmethyl clobazam.
Clobazam oxidation metabolism can affect disulfiram or cimetidine resulting in an increased plasma benzodiazepine levels.
Anticholinergic effects of other drugs, including atropine and similar drugs, antihistamines and antidepressants can be strengthened.
Patients must be reminded of possible interactions, especially with alcohol, such as the bioavailability of clobazam can increase by 50%.
Use of clobazam together with narcotic analgesics, increases the likelihood of "euphoria", and can bring psychological dependence.
The effects of muscle relaxants and nitrous oxide will be increased by clobazam.
