Indications/Uses
Adjunctive therapy of partial seizures w/ & w/o secondary generalization in adult & childn ≥6 yr. Monotherapy of partial seizures w/ & w/o secondary generalization in adult & childn ≥12 yr. Peripheral neuropathic pain eg, painful diabetic neuropathy & post-herpetic neuralgia in adult.
Dosage/Direction for Use
Epilepsy Adult & adolescent 900-3,600 mg/day.  Initially 300 mg on the 1st day of treatment, 300 mg bid on the 2nd day & 300 mg tid on the 3rd day. Thereafter, may increase dose in increments of 300 mg every 2-3 days until effective antiepileptic control is achieved which is usually w/ in the range of 0.9-3.6 g daily. Max: 4.8 g daily in 3 equally divided doses. Max time interval between doses: 12 hr. Peripheral neuropathic pain Adult 300 mg once daily in day, 300 mg bid in day 2 & 300 mg tid in day 3. Alternatively, 900 mg/day as 3 equally divided doses. Patient w/ renal impairment: CrCl ≥80 mL/min 900-3,600 mg/day in 3 divided doses, 50-79 mL/min 600-1,800 mg daily in 3 divided doses, 30-49 mL/min 300-900 mg daily in 3 divided doses, 15-29 mL/min 300 mg on alternate days to 600 mg daily in 3 divided doses, <15 mL/min 300 mg on alternate days to 300 mg daily in 3 divided doses. Patient undergoing hemodialysis Loading dose: 300-400 mg, followed by 200-300 mg after each 4-hr hemodialysis.
Administration
May be taken with or without food: Swallow whole w/ sufficient fluid.
Contraindications
Special Precautions
Drug rash w/ eosinophilia & systemic symptoms. Anaphylaxis. Suicidal ideation & behaviour. Discontinue if patient develops acute pancreatitis. Patients w/ mixed seizures including absences. Somnolence, sedation & resp depression w/ concomitant use w/ opioids. Resp depression in patients w/ compromised resp function, resp or neurological disease, renal impairment. Pregnancy & lactation. Childn. Elderly >65 yr.
Adverse Reactions
Viral infection. Somnolence, dizziness, ataxia.
Drug Interactions
Somnolence, sedation & resp depression w/ opioids. Reduce bioavailability w/ antacids containing Al & Mg.
