Prophylaxis for VTE during surgical procedures Patient at low to moderate risk 20 mg SC once daily, 1st dose approx 2 hr pre-op.
Patient at high risk 40 mg SC once daily, 1st dose approx 12 hr pre-op or 30 mg SC bid, starting w/in 12-24 hr post-op. Treatment is continued for 7-10 days or until patient is ambulant. May be continued at 40 mg once daily for further 3 wk following hip replacement surgery.
Childn >2 mth 0.5 mg/kg 12 hrly.
Infant <2 mth 0.75 mg/kg 12 hrly.
Prophylaxis of thromboembolism in immobilized medical patients 40 mg once daily for a min of 6 days & continued until full ambulation. Max: 14 days.
Treatment of DVT 1 mg/kg SC 12 hrly or 1.5 mg/kg once daily for 5 days.
Prevention of clotting in extracorporeal circulation during hemodialysis 1 mg/kg introduced into the arterial line of the circuit prior to dialysis. A further dose of 0.5-1 mg/kg may be given.
Treatment of unstable angina 1 mg/kg SC 12 hrly w/ low-dose aspirin for 2-8 days.
Treatment of thromboembolism Childn >2 mth 1 mg/kg 12 hrly.
Infant <2 mth 1.5 mg/kg 12 hrly.
