Nicardipine HCl
Indications/Uses
Management of HTN & angina pectoris.
Dosage/Direction for Use
Administer by continuous IV infusion only. Dilute to conc of 0.1-0.2 mg/mL before use unless given by central venous line. Adult Initially at rate of 3-5 mg/hr for 15 min continuous administration. Rates can be increased by increments of 0.5 or 1 mg every 15 min. Infusion rate should not exceed 15 mg/hr. Maintenance dose: Reduce dose progressively, usually to between 2 & 4 mg/hr, when target pressure is reached to maintain therapeutic efficacy. Childn Initially 0.5-5 mcg/kg/min in case of emergency. Maintenance dose: 1-4 mcg/kg/min. Elderly, hepatic & renal impairment, pregnancy Initially 1-5 mg/hr continuous infusion, depending on BP & clinical situation. Increase or decrease rate by increments of 0.5 mg/hr after 30 min depending on effect observed. Rate should not exceed 15 mg/hr.
Contraindications
Hypersensitivity. Severe aortic stenosis; compensatory HTN ie, in case of arteriovenous shunt or aortic coarctation; unstable angina; w/in 8 days after MI. Not to be taken by patients w/ rare hereditary problems of fructose intolerance.
Special Precautions
Control speed of administration accurately by use of electronic syringe driver or volumetric pump. Monitor BP & heart rate at least every 5 min during infusion, & then until vital signs are stable, but at least for 12 hr after end of administration. Hepatic & renal impairment. Pregnancy. Not to be used during lactation. Low birth wt infants, newborns, nursing infants, infants & childn. Only use for life-threatening HTN in ped intensive care settings or post-op contexts. Childn w/ renal impairment.
Adverse Reactions
Headache. Dizziness; swollen legs or ankles, increased heart rate, palpitations; low BP, especially on standing up, which may cause lightheadedness or fainting; feeling sick or being sick, flushing of skin.
