Indications/Uses
Source of calories & essential fatty acids for patient requiring parenteral nutrition for extended periods of time (usually >5 days) & as a source of essential fatty acids for prevention of essential fatty acids deficiency.
Dosage/Direction for Use
Administer as part of IV nutrition via peripheral vein or by central venous infusion. Adult Initial rate of infusion is 0.5 mL/min for the 1st 15-30 min, may increase to 1 mL/min if no untoward reactions occur. Not >500 mL should be infused on the 1st day of therapy. Max: 2.5 g of fat/kg (12.5 mL/kg). Childn Initial rate of infusion should be not >0.05 mL/min for the 1st 10-15 min. Rate can be changed to 0.5 mL/kg/hr if no untoward reactions occur. Max: 3 g of fat/kg/day. Premature infant 0.5 g fat/kg/24 hr (2.5 mL), may be increased in relation to the infant's ability to eliminate fat. Max: 3 g fat/kg/24 hr.
Contraindications
Patients w/ disturbances of normal fat metabolism eg, pathologic hyperlipidemia, lipid nephrosis or acute pancreatitis if accompanied by hyperlipidemia.
Special Precautions
Allergy to soy protein. Patients w/ fat metabolism hypofunction eg, hepatic & renal insufficiency, diabetic ketoacidosis, pancreatitis, hypothyroidism accompanied by hyperlipidemia & septicemia. Exam the fat clearance capacity if used >1 wk. May interfere w/ other lab test eg, bilirubin, lactate dehydrogenase, oxygen saturation, Hg if not completely cleared in the bloodstream. Carefully monitor the infant's ability to eliminate the infused fat from the circulation. Pregnancy & Lactation. Newborns & premature infants accompanied by hyperbilirubinemia or patients w/ suspicious pulmonary HTN.
Adverse Reactions
Increased body temp; cold fits, nausea & vomiting occasionally.
