Indications/Uses
Conditions where inhibition of gastric acid secretion may be beneficial including aspiration syndromes, dyspepsia, GERD, PUD & Zollinger-Ellison syndrome.
Dosage/Direction for Use
Cap Dyspepsia 10-20 mg daily for 2-4 wk. GERD Adult 20 mg once daily for 4-12 wk, followed by a further 4-6 wk if not fully healed, thereafter maintenance therapy w/ 20 mg once daily. Childn 0.7-1.4 mg/kg up to max daily dose of 40 mg for 4-12 wk. Refractory esophagitis 40 mg. Maintenance therapy: 20 mg once daily. PUD 20 mg as single daily dose or 40 mg for severe cases. Continue treatment for 4 wk for duodenal ulcers & 8 wk for gastric ulcer. Maintenance: 10-20 mg once daily. Zollinger-Ellison syndrome Initially 60 mg daily, adjusted as required up to 120 mg daily. Doses >80 mg daily should be divided in 2 doses. NSAID-associated ulceration 20 mg daily. Prophylaxis in patient w/ history of gastroduodenal lesions requiring continued NSAID treatment 20 mg daily. Inj Administer IV inj over 5 min or IV infusion over 20-30 min. Prophylaxis of acid aspiration 40 mg completed 1 hr before surgery. Gastric (benign) & duodenal ulcer, GERD 40 mg once daily until oral administration is possible. Severe peptic ulcer bleeding Initially 80 mg IV infusion then by continuous IV infusion 8 mg/hr for 72 hr (then change to oral).
Administration
Should be taken with food: Take immediately before a meal.
Contraindications
Hypersensitivity.
Special Precautions
Hepatic impairment. Cap: Pregnancy & lactation. Childn. Inj: Consider possibility of malignancy in patient w/ gastric ulcer.
Adverse Reactions
GI disturbances, in particular diarrhoea, skin rashes & headache; CNS effects, including reversible confusional states, agitation, depression & hallucinations in severely ill patient; flatulence. Cap: Nausea, constipation. Increased risk of enteric infections due to reduced acid secretion. Inj: Pruritus, dizziness, fatigue, constipation, nausea & vomiting, abdominal pain, arthralgia & myalgia, urticaria & dry mouth; insomnia, somnolence, vertigo.
Drug Interactions
May alter metabolism of some drugs metabolized by CYP450 system primarily by isoenzyme CYP2C19. May prolong elimination of diazepam, phenytoin & warfarin. May reduce absorption of drugs eg, ketoconazole & possibly itraconazole. Cap: May increase plasma levels of clarithromycin & 14-hydroxy-clarithromycin.
