Monotherapy, combination therapy w/ metformin, TZD or insulin (w/ or w/o metformin) 50 or 100 mg daily. Dual combination w/ sulfonylurea 50 mg once daily. Triple combination w/ metformin & sulfonylurea 100 mg once daily. 50-mg dose Administer once daily in the morning. 100-mg dose Administer as 2 divided doses of 50 mg given in the morning & evening.
Patients w/ type 1 diabetes or for the treatment of diabetic ketoacidosis; patients w/ hepatic impairment, pre-treatment ALT or AST >2.5 x the upper limit of normal, NYHA class III. Pregnancy & lactation. Childn <18 yr.
