Indications/Uses
Management of HTN & treatment of diabetic nephropathy.
Dosage/Direction for Use
Adult HTN 50 mg once daily. May be increased to 100 mg daily as a single dose or in 2 divided doses. Patient w/ intravascular fluid depletion Initially 25 mg once daily. Heart failure Initially 2.5 mg once daily. May be increased by 1.25 mg at wk interval to 50 mg once daily or as patient can tolerate.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Pregnancy (2nd & 3rd trimester).
Special Precautions
Discontinue when pregnancy is diagnosed. Renal artery stenosis. Hypotension in patients w/ vol depletion (eg, those who have received high-dose diuretic therapy). Correct vol depletion before starting therapy or use a low initial dose. Hyperkalemia; monitor serum K conc. Avoid concomitant use of K-sparing diuretics. Diabetic patients w/ reduced awareness of hypoglycemia. Renal & hepatic impairment. Not recommended during pregnancy (1st trimester) & lactation. Elderly.
Adverse Reactions
Hepatitis (rarely); malaise; thrombocytopenia (rarely); angioedema, including swelling of larynx & glottis, causing airway obstruction &/or swelling of face, lips, pharynx, &/or tongue, vasculitis including Henoch-Schönlein purpura, anaphylactic reactions; hyperkalemia, hyponatremia; rhabdomyolysis; dysgeusia; dry cough; erythroderma.
Drug Interactions
May increase hypotensive action w/ other antihypertensive agents. May induce & increase risk of hypotension w/ TCAs, antipsychotics, baclofen & amifostine.
