AdultHTN 50 mg once daily. May be increased to 100 mg daily as a single dose or in 2 divided doses. Patient w/ intravascular fluid depletion Initially 25 mg once daily. Heart failure Initially 2.5 mg once daily. May be increased by 1.25 mg at wk interval to 50 mg once daily or as patient can tolerate.
Discontinue when pregnancy is diagnosed. Renal artery stenosis. Hypotension in patients w/ vol depletion (eg, those who have received high-dose diuretic therapy). Correct vol depletion before starting therapy or use a low initial dose. Hyperkalemia; monitor serum K conc. Avoid concomitant use of K-sparing diuretics. Diabetic patients w/ reduced awareness of hypoglycemia. Renal & hepatic impairment. Not recommended during pregnancy (1st trimester) & lactation. Elderly.
