Indications/Uses
Treatment of skeletal events in patients w/ advanced bone malignancies. Treatment of hypercalcemia of malignancy, Paget's disease of bone & osteoporosis in postmenopausal women.
Dosage/Direction for Use
Administered as IV infusion over not <15 min. Hypercalcemia of malignancy 4 mg as single dose (diluted w/ 100 mL of NaCl 0.9% or glucose 5%). Treatment may be repeated if necessary after at least 7 days at a dose of 4 mg. Max: 4 mg. Prevention of skeletal events in patients w/ advanced bone malignancies 4 mg (diluted w/ 100 mL NaCl 0.9% or glucose 5%) given every 3-4 wk. Treatment of Paget's disease of bone 5 mg as single IV infusion. Treatment of osteoporosis in postmenopausal women Single dose.
Contraindications
Hypersensitivity to zoledronic acid or other bisphosphonates.
Special Precautions
Ensure patients are adequately hydrated. Avoid overhydration in patients at risk of cardiac failure. Carefully monitor serum levels of Ca, phosphate & Mg after initiation of therapy. Patients w/ mild to moderate renal impairment, severe hepatic insufficiency. Monitor for severe & occasionally incapacitating bone, joint &/or muscle pain; atypical femur fractures.
Adverse Reactions
Flu-like syndrome: Fever, chills, bone pain &/or arthralgia.
Drug Interactions
Potential additive effect w/ aminoglycosides. Other potentially nephrotoxic drugs. Increased risk of renal dysfunction (in multiple myeloma patients) w/ thalidomide. Risk of osteonecrosis of the jaw w/ anti-angiogenic drugs.
