Vancomycin HCl
Indications/Uses
Severe, potentially life-threatening Staph infections in patients who cannot receive or failed to respond w/ penicillins or cephalosporins. Bone & joint infections (including osteomyelitis), pneumonia, septicemia, skin & soft tissue infections & endocarditis caused by susceptible strains of Staph sp (including methicillin-resistant strains). Primary agent as monotherapy or combination w/ aminoglycoside or rifampin in endocarditis caused by Corynebacterium sp (diphtheroids) (including penicillin- & cephalosporin-resistant strains) in penicillin-allergic patients, & as secondary agent in endocarditis caused by Strep viridians or Strep bovis. Prophylaxis of bacterial endocarditis in penicillin-allergic patients w/ prosthetic heart valves or congenital, rheumatic, or other acquired valvular heart disease who are undergoing dental or surgical procedures of upper resp tract.
Dosage/Direction for Use
IV infusion Administer over 60 min or longer by intermittent IV infusion, or over 24-hr by slow IV drip. Adult w/ normal renal function 1 g or 10-15 mg/kg every 12 hr. Prophylaxis for bacterial endocarditis in dental, oral or upper resp tract surgery 1 g 1 hr before surgery, GI/GU procedure 1 g + 1.5 mg/kg gentamicin 1 hr prior surgery. Childn & infant >1 mth 40 mg/kg daily in divided doses every 6 hr. Staph CNS infection 60 mg/kg daily in divided doses every 6 hr. Prophylaxis for bacterial endocarditis in dental, oral, or upper resp tract surgery 20 mg/kg 1 hr prior to procedure, GI/GU procedure 20 mg/kg + gentamicin 2 mg/kg 1 hr prior to surgery. Neonate: Postnatal age >7 days weighing ≥1,200 g 10 mg/kg divided every 8 hr, <1,200 g 15 mg/kg every 24 hr. Postnatal age ≤7 days weighing >2,000 g 15 mg/kg every 12 hr, 1,200-2,000 g 10 mg/kg every 12 hr, <1,200 g 15 mg/kg every 24 hr. Renal impairment w/ CrCl >60 mL/min Initially 1 g or 10-15 mg/kg every 12 hr, CrCl 40-60 mL/min Initially 1 g or 10-15 mg/kg every 24 hr.
Contraindications
Hypersensitivity. Patients w/ previous severe hearing loss.
Special Precautions
Patients receiving other nephrotoxic or ototoxic drugs. Renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Hypotension accompanied by flushing; erythematous rash on face & upper body (red neck or red man syndrome). Chills, drug fever; eosinophilia. Ototoxicity, renal failure, thrombocytopenia.
Drug Interactions
Additive toxicity w/ concurrent or sequential use of ototoxic &/or nephrotoxic drugs (eg, aminoglycosides, amphotericin B, bacitracin, cisplatin, colistin, polymyxin B) & vancomycin.
