Indications/Uses
In combination w/ corticosteroids &/or other immunosuppressive agents to enhance survival of organ transplants & reduce corticosteroid requirements of renal transplant recipients. Alone or in combination w/ corticosteroids for severe RA, SLE, dermatomyositis & polymyositis, autoimmune chronic active hepatitis & haemolytic anemia, pemphigus vulgaris, polyarteritis nodosa, chronic refractory idiopathic thrombocytopenic purpura.
Dosage/Direction for Use
Adult & childn Transplantation Up to 5 mg/kg daily on the 1st day. Maintenance: 1-4 mg/kg daily. Other conditions Initially 1-3 mg/kg daily. Maintenance: <1-3 mg/kg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to azathioprine & mercaptopurine. Pregnancy & lactation.
Special Precautions
Hypersensitivity. Patient w/ infection; Lesch-Nyhan syndrome; thiopurine methyltransferase (TPMT) enzyme deficiency. Increased fertility in renal transplant recipients. Signs or symptoms of bone marrow suppression eg, inexplicable bruising or bleeding, infection. Monitor blood counts & adjust dosage for bone-marrow toxicity. Avoid sunlight exposure in patients w/ increased risk of skin cancer. Lactose intolerance. Renal or liver impairment. Women of childbearing potential. Not to be given during pregnancy. Not recommended during lactation.
Adverse Reactions
Viral, fungal & bacterial infections; benign & malignant neoplasms including lymphomas, skin cancers, AML & myelodysplasia; bone marrow depression, leucopenia, thrombocytopenia, anaemia, agranulocytosis, pancytopenia, aplastic or megaloblastic anaemia, erythroid hypoplasia; reversible pneumonitis; pancreatitis, colitis, diverticulitis & bowel perforation, severe diarrhoea, nausea; cholestasis, life-threatening hepatic damage; alopecia; hypersensitivity reactions.
Drug Interactions
Concomitant use w/ allopurinol, oxipurinol, thiopurinol, succinylcholine, tubocurarine, warfarin, cytostatics, peniclllamine, ACE inhibitors (captopril), cimetidine, indomethacin, vaccines, olsalazine, mesalazine, sulphasalazine, frusemide. Atypical & potentially deleterious response to live vaccines.
