Atracurium besylate
Indications/Uses
Adjunct to general anesth to facilitate endotracheal intubation & relax skeletal muscles during surgery; to facilitate mechanical ventilation in ICU patients.
Dosage/Direction for Use
Adult IV 0.3-0.6 mg/kg. Endotracheal intubation 0.5-0.6 mg/kg w/in 90 sec. Full block prolongation 0.1-0.2 mg/kg. IV infusion After an initial 0.3-0.6 mg/kg bolus dose, neuromuscular block may be maintained by administration of 0.3-0.6 mg/kg/hr as continuous infusion. ICU patients Maintenance of neuromuscular block: 11-13 mcg/kg/min. Childn >1 mth Adult dose based on body wt.
Contraindications
Special Precautions
Patients w/ history of increased sensitivity to histamine effects; myasthenia gravis; burns. Hypovolaemic patients. Not to be administered into the infusion line of blood transfusion. Pregnancy & lactation.
Adverse Reactions
Skin flushing, mild transient hypotension or bronchospasm.
Drug Interactions
Concomitant use w/ inhalation anesth eg, halothane, isoflurane & enflurane; antibiotics eg, aminoglycosides, polymyxin, spectinomycin, tetracyclines, lincomycin & clindamycin; antiarrhythmics eg, propranolol, Ca channel blockers, lignocaine, procainamide & quinidine; diuretics eg, furosemide, mannitol, thiazides & acetazolamide; Mg sulfate; ketamine; lithium salts; ganglion-blocking agents eg, trimetaphan, hexamethonium.
