Action
Lansoprazole is an anti-secretion that works to suppress gastric acid secretion by inhibiting the activity of the H/K ATPase (proton pump) enzyme on the glandular surface of parietal gastric cells at pH <4. Lansoprazole which binds to proton (H) rapidly will be converted to sulfonamide, an active proton pump inhibitor. Oral use of Lansoprazole inhibits secretion of basal gastric acid and penta-gastric stimulation.
Indications/Uses
For the treatment of duodenal ulcers, gastric ulcers and reflux esophagitis.
Dosage/Direction for Use
Duodenal ulcers and esophagitis ulcers: 30 mg once daily for 4 weeks, taken in the morning before meal. Gastric ulcer: 30 mg once daily for 8 weeks, taken in the morning before meal. Patients with liver function impairment: The dose is not more than 30 mg daily.
Contraindications
Hypersensitivity.
Special Precautions
Possibility of malignancy in the stomach must be removed before treatment is given, because treatment with Lansoprazole may cover the symptoms of malignancy so that the diagnosis is delayed. Should be taken in the morning before meals. Capsules should not be opened, chewed or destroyed and must be completely swallowed. Caution in use for pregnant women, breastfeeding women or children. Breastfeeding must be stopped if it is really deemed necessary to get Lansoprazole treatment.
Adverse Reactions
Lansoprazole is generally well tolerated. At large doses and prolonged use it may stimulate ECL cell growth (enterochromaffin-like-cells). In long-term use, the patient need to be cautious of the excessive growth of bacteria in digestive system.
Sometimes side effects that may occur such as headache, diarrhea, abdominal pain, dyspepsia, nausea, vomiting, dry mouth, constipation, bloating, dizziness, skin rash, urticaria and pruritus.
There is a temporary elevation in liver function test values and will be normal again.
Occasional arthralgia, peripheral edema and depression.
Although it is rare, it has been reported that there are changes in hematological numbers (such as thrombocytopenia, eosinophilia, leukopenia).
Drug Interactions
Lansoprazole can prolong elimination of drugs metabolized by cytochrome P450 in the liver, such as Diazepam, Warfarin and Phenytoin. Monitoring of patients who also get Warfarin and Phenytoin treatment is highly recommended, it may be necessary to reduce the dose of Warfarin or Phenytoin. Giving along with oral contraceptives and preparations such as Phenytoin, Theophylline, or Warfarin and Antacids and Sucralfate will reduce the bioavailability of Lansoprazole, therefore should not be given one hour after taking Lansoprazole. Lansoprazole interferes with the absorption of drugs whose absorption is affected by gastric pH such as Ketoconazole, Esters, Ampicillin, iron salt.
