Dexmedetomidine HCl
Indications/Uses
Sedation of initially intubated & mechanically ventilated patients in ICU; non-intubated patients prior to &/or during surgical & other procedures.
Dosage/Direction for Use
IV Adult ICU sedation Loading dose of 1 mcg/kg over 10 min, followed by maintenance infusion 0.2-0.7 mcg/kg/hr. May be adjusted in increments of 0.1 mcg/kg/hr or higher to achieve desired level of sedation if after approx 5 min sedation is not adequate. Procedural sedation Loading dose of 1 mcg/kg over 10 min. For less invasive procedures, loading dose of 0.5 mcg/kg over 10 min. Maintenance dose: Initiate at 0.6 mcg/kg/hr, may be titrated from 0.2-1 mcg/kg/hr until desired clinical effect is achieved. Awake fiberoptic intubation 0.7 mcg/kg/hr until endotracheal tube is secured. Elderly >65 yr Loading dose: 0.5 mcg/kg over 10 min.
Contraindications
Hypersensitivity.
Special Precautions
Pre-existing severe bradycardia disorders (ie, advanced heart block), or severe ventricular dysfunction (eg, ejection fraction <30%) including CHF & cardiac failure. Patients w/ desensitized autonomic nervous system control. Hypotension in hypovolemic patients. Transient HTN primarily during loading dose infusion associated w/ initial peripheral vasoconstrictive effects. Bradycardia & sinus arrest may occur. May cause reduced lacrimation. Monitor ECG, BP & O2 saturation during infusion. Inspected visually for particulate matter & discoloration prior to administration. W/drawal symptoms may occur if administered chronically (>24 hr) & stopped abruptly. Do not administer >24 hr. Pregnancy & lactation. Elderly >65 yr.
Adverse Reactions
Hypotension, HTN, bradycardia, dry mouth & nausea. Drug abuse & dependence. Supraventricular tachycardia, visual impairment, chills, hyperpyrexia, pyrexia, hyperbilirubinemia, ECG QT prolonged, hypernatremia & urticaria.
Drug Interactions
Enhanced effects w/ anesth, sedatives, hypnotics & opioids. Increased pharmacodynamic effects w/ vasodilators or -ve chronotropic agents. May potentiate clinical events of bradycardia or hypotension w/ propofol or midazolam.
