Vancomycin HCl
Indications/Uses
Severe, potentially life-threatening infections caused by susceptible gm +ve microorganisms eg, endocarditis; bone infections (osteomyelitis); pneumonia; soft-tissue infections. Perioperative prophylaxis against bacterial endocarditis in major surgical procedures (eg, cardiac & vascular procedures).
Dosage/Direction for Use
Administered at a rate not exceeding 10 mg/min or at least 60 min. Adult & adolescent >12 yr 2,000 mg IV in 4 divided doses every 6 hr or 1,000 mg IV every 12 hr. Bacterial endocarditis 1,000 mg IV every 12 hr for 4 wk as monotherapy or in combination w/ other antibiotics (eg, gentamycin + rifampin, gentamicin, streptomycin). Enterococcal endocarditis Duration: 6 wk in combination w/ aminoglycoside. Peri-operative prophylaxis against bacterial endocarditis Adult 1,000 mg IV prior to surgery (before induction of anesth) followed by 1,000 mg IV every 12 hr post-op. Childn 1 mth-12 yr 10 mg/kg every 6 hr or 20 mg/kg every 12 hr. Infant & newborn 15 mg/kg, followed by 10 mg/kg every 12 hr during 1st wk of life & every 8 hr after & up to 1 mth. Mild to moderate renal failure Starting dose: 15 mg/kg. Severe renal failure Maintenance dose: 250-1,000 mg at several days interval. Anuria Starting dose: 15 mg/kg until therapeutic serum conc is reached. Maintenance: 1.9 mg/kg daily. Haemodialysis 1,000 mg IV saturating dose & 1,000 mg maintenance dose IV every 7-10 days. If polysulfone membranes are used, additional maintenance dose may be necessary.
Contraindications
Special Precautions
Inspect soln visually for particulate matter or discoloration before administration. Administer separately in case of combination therapy w/ other antibiotics/chemotherapeutics. Flush IV lines before administration of antibiotics. Hepatic impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Decreased BP, thrombophlebitis; dyspnoea, stridor; exanthema & mucosal inflammation, pruritus, urticaria; renal insufficiency manifested primarily by increased serum; upper body & face redness, pain & spasm of chest & back muscles.
Drug Interactions
May potentiate nephrotoxicity &/or ototoxicity w/ gentamycin, amphotericin B, streptomycin, neomycin, kanamycin, amikacin, tobramycin, viomycin, bacitracin, polymyxin B, colistin & cisplatin. Erythema, histamine like flushing & anaphylactoid reactions w/ anesth agents. Enhanced & prolonged effect of muscle relaxant (eg, succinylcholine).
