Quetiapine fumarate
Indications/Uses
Schizophrenia & bipolar disorder.
Dosage/Direction for Use
Schizophrenia Initially 50 mg daily on day 1, increased in increments of 50-150 mg as tolerated on days 2 & 3 to a target of 300-400 mg daily by day 4. Daily dose on day 1 is given in 2 divided doses but may be given in 3 divided doses thereafter. Daily dose may be further adjusted as necessary in steps of 50-100 mg at intervals of not <2 days to max daily dose of 750 mg. Acute manic episodes associated w/ bipolar disorder Initially 50 mg bid on day 1, 100 mg bid on day 2, 150 mg bid on day 3 & 200 mg bid on day 4. Dose may then be adjusted to 400-800 mg daily according to response. Increments should be no >200 mg daily. Depressive phase of bipolar disorder Initially 50 mg once daily at bedtime, increased to 100 mg on day 2, 200 mg on day 3 & 300 mg on day 4. Dose may be further increased to 400 mg on day 5 & 600 mg on day 8 if necessary. Maintenance treatment of bipolar disorder as adjunct to lithium or valproate Continue dose that controlled initial symptoms. Elderly & hepatic impairment Initially 25 mg daily, may be increased in increments of 25-50 mg daily according to response.
Administration
May be taken with or without food: Swallow whole, do not split/chew/crush.
Contraindications
Hypersensitivity. Concomitant use w/ cytochrome P450 3A4 inhibitors eg, HIV-PIs, azole antifungal agents, erythromycin, clarithromycin & nefazodone.
Special Precautions
Asymptomatic changes in lens of eye during long-term treatment. Patients w/ CV disease or other conditions predisposing to hypotension, w/ cerebrovascular disease or history of seizures or conditions lowering seizure threshold. Closely monitor patients treated for depressive phase in bipolar disorder during early therapy until significant improvement in depression is observed because of the inherent risk of suicide in depressed patients. Gradual w/drawal is recommended. May interfere w/ activities requiring mental alertness. Hepatic or renal impairment. Pregnancy & lactation. Elderly w/ dementia.
Adverse Reactions
Somnolence & dizziness; mild asthenia, anxiety, fever, rhinitis, peripheral edema, constipation, dyspepsia, dry mouth & raised liver enzyme value.
Drug Interactions
May enhance central effects of other CNS depressants including alcohol. Concomitant use w/ antihypertensives or drugs prolonging QT interval; CYP3A4 potent inhibitors eg, erythromycin, fluconazole, itraconazole & ketoconazole. May antagonise actions of dopaminergics eg, l-dopa. Plasma conc may be decreased w/ CYP3A4 inducers eg, carbamazepine & phenytoin. Increased clearance w/ thioridazine. Inhibited metabolism w/ antibacterials eg, erythromycin. Asymptomatic QT prolongation w/ risperidone.
