Vancomycin HCl
Indications/Uses
Serious staphylococcal or other Gram +ve infections where other drugs (eg, penicillins) cannot be used because of resistance or patient intolerance. Methicillin-resistant staphylococcal infections in conditions eg, brain abscess, staphylococcal meningitis, peritonitis associated w/ continuous ambulatory peritoneal dialysis & septicaemia. Alone or in combination w/ another drug (eg, aminoglycoside) in treatment & prophylaxis of endocarditis, prophylaxis of surgical infection, & in intensive care & management of immunocompromised patients.
Dosage/Direction for Use
Administer IV by intermittent (over at least 60 min for 500-mg or 100 min for 1-g) or continuous (over 24 hr) infusion. Adult 500 mg every 6 hr or 1 g every 12 hr. Staphylococcal endocarditis Duration: At least 3 wk. High-risk penicillin-allergic patient undergoing dental or other procedure Prophylaxis of endocarditis 1 g single dose by IV infusion over at least 100 min followed by IV gentamicin before the procedure. Childn & infant >4 wk 10 mg/kg every 6 hr. Neonate & infant up to 4 wk Initially 15 mg/kg followed by 10 mg/kg every 12 hr in the 1st wk of life or by 10 mg/kg every 7 hr in those aged 1-4 wk. Prophylaxis of endocarditis Single dose mg/kg.
Special Precautions
Hypersensitivity. Not to be given IM. Risk of tissue necrosis.
Adverse Reactions
Red-neck or red-man syndrome; hypersensitivity reactions; reversible neutropenia, eosinophilia, & rarely thrombocytopenia & agranulocytosis; nephrotoxicity.
Drug Interactions
Increased risk of toxicity w/ other ototoxic or nephrotoxic drugs eg, aminoglycosides, polymyxins & loop diuretics. Some adverse effects may be enhanced w/ general anaesth. May increase neuromuscular blockade of suxamethonium or vecuronium.
