Unstable angina/non-Q-wave MI 1 mg/kg SC bid for 2-8 days.
Acute STEMI W/ a thrombolytic agent, initially 30 mg IV bolus followed by 1 mg/kg SC w/in 15 min then 12 hrly for 8 days or until discharged. Max: 10,000 anti-Xa IU for the 1st 2 SC doses.
Patient ≥75 yr 0.75 mg/kg 12 hrly. Max: 7,500 anti-Xa IU for the 1st 2 inj only. Do not administer an initial IV bolus dose.
Prophylaxis of thromboembolic disorders Low to moderate risk: In general surgery, 20 mg SC once daily for 7-10 days. In patients undergoing surgery, give initial dose approx 2 hr pre-op. High risk: In orthopedic surgery, 40 mg SC once daily for 7-10 days, give initial dose approx 12 hr pre-op. In bedridden medical patients: 40 mg SC once daily for 6-14 days.
Treatment of thromboembolic disease 1 mg/kg SC bid for 10 days or 1.5 mg/kg SC once daily for 10 days.
Hemodialysis 0.5-1 mg/kg for a 4-hr session.
