Levofloxacin hemihydrate
Indications/Uses
Mild to moderate infections caused by gm +ve & -ve bacteria including anaerobes.
Dosage/Direction for Use
Administer by slow IV infusion over 60-90 min. Patient w/ normal renal function 250-750 once daily for 7-14 days depending on severity of disease.
Contraindications
Hypersensitivity to levofloxacin or other quinolones.
Special Precautions
W/draw treatment if there is symptoms of ruptures of shoulder, hand & achilles tendons that required surgical repair/prolonged disability. Discontinue use at 1st appearance of skin rash or any sign of hypersensitivity. Increased ICP & central confusion, hallucinations, paranoia, depression, nightmares, insomnia, suicidal thoughts/acts. Patient w/ CNS disorders predispose to seizures; existing heart problems; arrhythmia; HTN or low blood K. Convulsions & toxic psychoses. Antibiotic-associated colitis. May cause phototoxicity reaction eg, TEN, SJS, erythema multiforme. May alter blood glucose level. Consider fluid & electrolytes management, protein supplementation & drug treatment clinically effective against C. difficile colitis. Avoid in concomitant use w/ drugs prolonging QT interval eg, quinidine, procainamide, amiodarone, cisapride, erythromycin, antipsychotics, TCAs. Increased pharmacological action of warfarin. Renal impairment CrCl <50 mL/min. Not to be used during pregnancy & lactation. Adolescents <18 yr & childn.
Adverse Reactions
Nausea, diarrhea, constipation, insomnia, headache, inj site reaction.
Drug Interactions
Concomitant use w/ NSAIDs, antidiabetic agents. Antacids containing Al or Mg or drugs containing Fe.
