Levofloxacin hemihydrate
Indications/Uses
Pneumonia, complicated urinary infections (including pyelonephritis), skin & soft tissue infections, chronic bacterial prostatitis in adults.
Dosage/Direction for Use
IV infusion Adult Community-acquired pneumonia, skin & soft tissue infections 500 mg once daily to bid. Complicated urinary infections including pyelonephritis 250 mg once daily. Chronic bacterial prostatitis 500 mg once daily.
Contraindications
Hypersensitivity to levofloxacin or other quinolones. Epilepsy, complaints of tendon-related side effects caused by preliminarily used quinolones. Childn <18 yr.
Special Precautions
May not provide optimal therapeutic effect in very severe pneumonia. May require combined therapy in hospital acquired diseases caused by P. aeruginosa. Discontinue use if decreased BP was observed during administration; tendinitis, pseudomembranous colitis is suspected; seizures, serious potentially fatal hypersensitivity & psychotic reactions occur. Patients taking corticosteroids; disposed to seizures eg, patients w/ CNS disturbances in history, at concomitant therapy w/ fenbufen & similar NSAIDs or medicines that increase seizure activity (lower seizure threshold) eg, theophylline. Patients w/ G6PD; psychotic disorders or mental disorders in history; known risk factors for QT prolongation. Carefully monitor blood glucose levels in patients w/ DM receiving concomitant therapy w/ oral hypoglycemic agents (eg, glibenclamide) or insulin. Avoid sunlight or artificial UV radiation exposure. Monitor coagulation tests; concomitant use w/ vit K antagonists (eg, warfarin). Peripheral neuropathy. May provide false +ve result in determination of urine opiates. Hepatobiliary disorders. May affect ability to drive & operate machines. Renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Mycoses; leucopenia, eosinophilia, thrombocytopenia, neutropenia, agranulocytosis, pancytopenia, hemolytic anemia; anaphylactic shock, anaphylactic & anaphylactoid reactions (after the 1st dose), increased sensitivity; anorexia, hypoglycemia especially in patients w/ diabetes; insomnia, nervousness, psychotic disorders, depression, confused consciousness, anxiety, agitation, alarm, psychotic reactions w/ self-destructive behavior, including suicidal ideation or action, hallucinations; dizziness, headache, drowsiness; visual disturbances; vertigo, acoustic disturbance, tinnitus; tachycardia, prolongation of the QT interval on ECG; hypotension; bronchial spasms, dyspnea, allergic pneumonitis; diarrhea, nausea, vomiting, abdominal pains, dyspepsia, abdominal distention, constipation, hemorrhagic diarrhea; increased liver enzyme levels (ALT, AST, alkaline phosphatase, γ-glutamine transferase); rash, itching, urticaria, angioneurotic edema, hypersensitivity to sunlight & UV emission, TEN (Lyell's syndrome), SJS, exudative multiform erythema, hyperhidrosis; tendon injury including tendinitis, arthralgia, myalgia, tendon rupture; increased level of creatinine in blood serum, acute renal insufficiency; pain & redness in the place of infusion; phlebitis, asthenia, pyrexia, pain (including back, chest & limb pain).
Drug Interactions
Possible decrease of seizure threshold w/ theophylline, NSAIDs & other agents which reduce seizure threshold. High conc w/ fenbufen. Decreased renal clearance w/ probenecid & cimetidine. Increased t½ period of cyclosporine. Elevation of coagulation tests (PT/INR) &/or bleeding w/ vit K antagonists (eg, warfarin). Concomitant use w/ medicinal products that prolong QT interval (eg, class IA & III antiarrhythmics, TCAs & macrolides).
