Paracetamol
Indications/Uses
Management of mild to moderate pain; moderate to severe pain w/ adjunctive opioid analgesics. Fever reduction.
Dosage/Direction for Use
Administer as 15-min IV infusion. Adolescent & adult >50 kg 1 g up to qid or 650 mg every 4 hr. Max: 4 g daily, min of 4 hr dosing interval. Childn, adolescent & adult <50 kg 15 mg/kg up to qid or 12.5 mg/kg every 4 hr. Max: 75 mg/kg daily, min of 4 hr dosing interval. Severe renal impairment (CrCl 30 mL/min) Increase min interval to 6 hr. Adult w/ hepatocellular insufficiency, chronic alcoholism & malnutrition & dehydration Max daily dose: Not to exceed 3 g.
Special Precautions
Risk of very serious liver damage hepatic injury (including liver failure & death) in doses higher than recommended. Pregnancy.
Adverse Reactions
Hepatic injury, serious skin reactions (eg, acute generalised exanthematous pustulosis, SJS, TEN), allergy & hypersensitivity (eg, swelling of the face, mouth & throat, resp distress, urticaria, rash & pruritus). Nausea, vomiting; pyrexia; headache; insomnia. Anemia; muscle spasms; HTN, hypotension. Adult: Fatigue, infusion site pain, peripheral edema; AST increases, abnormal breath sounds; hypokalemia; anxiety; dyspnea. Childn: Tachycardia; abdominal pain, diarrhea; Inj site pain, peripheral edema; increase hepatic enzyme; hypoalbuminemia, hypokalemia, hypomagnesemia, hypophosphatemia, hypervolemia; pain in extremity; oliguria; pulmonary edema, hypoxia, pleural effusion, stridor, wheezing; periorbital edema.
Drug Interactions
Reduced clearance w/ probenecid. May prolong elimination w/ salicylamide.
