Cefepime HCl
Indications/Uses
Empiric monotherapy in febrile neutropenic patients. Infections due to susceptible organisms including suspected gm -ve organisms resistant to older cephalosporins (sepsis, pneumonia, meningitis) of the urinary tract, resp tract & skin.
Dosage/Direction for Use
Deep IM inj or IV by infusion over at least 30 min. Mild to moderate infections 1-2 g daily in 2 divided doses. Severe infections Increased to 4 g daily in 2 divided doses. Febrile neutropenia Up to 6 g daily in 3 divided doses. Childn >2 mth & ≤40 kg 50 mg/kg bid. Febrile neutropenia 50 mg/kg tid. Renal impairment CrCl 30-60 mL/min 0.5-2 g every 24 hr, 11-29 mL/min 0.5-1 g every 24 hr, ≤10 mL/min 0.25-0.5 g every 24 hr. Patients undergoing haemodialysis 1 dose after each dialysis sessions. Continuous ambulatory peritoneal dialysis Normal recommended doses at intervals of 48 hr.
Contraindications
Hypersensitivity to cefepime or other cephalosporins or penicillins.
Special Precautions
Penicillin-sensitive patients; history of allergy. Monitor renal & haematological status during prolonged & high-dose therapy. May interfere in creatinine conc measurement w/ Jaffe method & may produce falsely high values. Renal impairment. Pregnancy & lactation.
Adverse Reactions
Hypersensitivity reactions including skin rashes, urticaria, eosinophilia, fever, serum sickness-like reactions (w/ prolonged parenteral administration) & anaphylaxis. Neutropenia & thrombocytopenia. Transient increases in liver enzyme values. Convulsions & other signs of CNS toxicity (high doses). Pseudomembranous colitis (prolonged use). Pain at inj site (IM use) & thrombophlebitis (IV infusion).
Drug Interactions
May increase risk of kidney damage w/ nephrotoxic drugs eg, aminoglycosides gentamicin & tobramycin. Enhanced nephrotoxicity w/ loop diuretic furosemide. Inhibited renal excretion w/ probenecid. Antagonism w/ bacteriostatic antibacterials.
