Indications/Uses
Relief of pain & inflammation in musculoskeletal & joint disorders eg, RA, OA & ankylosing spondylitis; periarticular disorders eg, sprains & strains & other painful conditions eg, renal colic, acute gout, dysmenorrhea, migraine & following some surgical procedures. Actinic keratoses & fever.
Dosage/Direction for Use
IM/IV 75 mg once daily or bid (in severe conditions) by deep IM inj into the gluteal muscle. May also be given as continuous or intermittent IV infusion or as bolus IV inj. Post-op pain 75 mg over 30-120 min or as a bolus inj, may be repeated once after 4-6 hr. Prevention of post-op pain Initially 25-50 mg after surgery over 15-16 min followed by 5 mg/hr. Max: 150 mg daily. Alternatively, initial dose may be given as bolus inj over 5-60 sec followed by additional inj up to max daily dosage, may be repeated after 4-6 hr. Max daily doses: 150 mg. Max period: 2 days. Renal colic 75 mg repeated once after 30 min.
Contraindications
Hypersensitivity. History of stroke; CVA, heart attack (MI), CABG, uncontrolled HTN, CHF (NYHA II-IV). Asthma attacks, urticaria or acute rhinitis precipitated by ASA or other drugs w/ prostaglandin synthetase-inhibiting activity. Gastric or intestinal ulcer.
Special Precautions
GI disorders or history of gastric or intestinal ulceration, ulcerative colitis or Crohn's disease. Discontinue liver disease or other manifestations (eg, eosinophilia, rash) occur. Hepatitis; hepatic porphyria; cardiac, hepatic or renal impairment. Patients receiving diuretics, w/ extracellular vol depletion eg, major pre- & post-op. May temporarily inhibit platelet aggregation. Monitor renal & hepatic function; blood count; patients w/ defective hemostasis. May affect ability to drive or operate machinery. Not recommended during pregnancy & lactation. Not suitable for childn. Elderly.
Adverse Reactions
Epigastric pain, nausea, vomiting, diarrhea, abdominal cramps, dyspepsia, flatulence, anorexia; headache, dizziness, vertigo; rashes; serum aminotransferase enzymes elevation.
Drug Interactions
May raise plasma conc of lithium & digoxin. May inhibit activity of diuretics; increased serum K levels w/ K-sparing diuretics. May increase frequency of adverse effects w/ NSAIDS. Increased risk of hemorrhage w/ anticoagulants. May increase toxicity of methotrexate & caution if used <24 hr before or after treatment. May increase nephrotoxicity of cyclosporine. Quinolones & NSAIDs.
