Methyldopa
Dosage/Direction for Use
Adult Initially, 250 mg bid-tid in the 1st 48 hr. Maintenance: 0.5-2 g/day in 2-4 divided doses. Max: 3 g/ day. Elderly Initially 125 mg bid. Max: 2 g/day. Childn Initially, 10 mg/kg in 2-4 divided doses. Max: 650 mg/kg or 3 g daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Acute hepatitis & active cirrhosis, liver disorders previously associated w/ methyldopa therapy, disease of pheochromocytoma. MAOIs. Lactation.
Special Precautions
Hypersensitivity to aspirin. Perform blood counts, Coombs' test & liver function test before initial therapy & at periodic intervals. Hemolytic anemia. Parkinsonism, mental depression, porphyria, cerebral atherosclerosis. History of previous liver disease or dysfunction. Severely impaired kidney. Allergic reaction to FD&C yellow #5 (tartrazine). Intention of driving or machinery operation.
Adverse Reactions
Sialoadenitis; hemolytic anemia, bone-marrow failure, leukopenia, granulocytopenia, thrombocytopenia, eosinophilia;  hyperprolactinemia; psychic disturbances including nightmares, reversible mild psychoses or depression, decreased libido; sedation (usually transient), headache, paraesthesia, parkinsonism, VIIth nerve paralysis, choreoathetosis, mental impairment, carotid sinus syndrome, dizziness, symptoms of cerebrovascular insufficiency; bradycardia, angina pectoris, myocarditis, pericarditis, AV block; orthostatic hypotension; nasal congestion; nausea, vomiting, abdominal distension, constipation, flatulence, diarrhoea, colitis, dry mouth, glossodynia, tongue discolouration, pancreatitis; liver disorders including hepatitis, jaundice; rash (eczema, lichenoid eruption), toxic epidermal necrolysis; lupus-like syndrome, mild arthralgia w/ or w/o joint swelling myalgia; breast enlargement, gynaecomastia, amenorrhea, lactation disorder, erectile dysfunction, ejaculation failure; asthenia, oedema (& wt gain) usually relieved by use of a diuretic (discontinue methyldopa if oedema progresses or signs of heart failure appear), pyrexia; +ve Coombs' test, +ve tests for antinuclear antibody, LE cells & rheumatoid factor, abnormal liver-function tests, increased blood urea.
Drug Interactions
Increased antihypertensive effect w/ other antihypertensives. Hypotension w/ anesth & MAOIs. Increased lithium toxicity. May antagonized antihypertensive effect & loss of control of BP w/ amphetamines, other CNS stimulants, TCAs. Decreased plasma conc & antihypertensive effect w/ Fe hematinic. Increased risk of vascular damage & difficulty in controlling BP w/ OCs.
