Indications/Uses
Gestation choriocarcinoma, chorioadenoma destruens, hydatidiform mole & acute lymphocytic leukemia alone or in combination w/ other chemotherapeutic agents. Symptomatic control of severe, recalcitrant, disabling psoriasis &/or dermatological consultation.
Dosage/Direction for Use
Given by IM, IV, intra-arterial or intrathecal route. Choriocarcinoma & similar trophoblastic disease 15-30 mg daily IM for 5 days w/ rest periods of >1 wk. Requires 3-5 courses. Leukemia 3.3 mg/m2 IV w/ prednisone 60 mg/m2 daily. Maintenance dose: 30 mg/m2 orally or IM twice wkly, or alternatively 2.5 mg/kg IV every 14 days. Meningeal leukemia Administer 12 mg/m2 intrathecally. Childn ≥3 yr 12 doses, 2 yr 10 doses, 1 yr 8 doses, <1 yr 6 doses. Mycosis fungoides 50 mg once wkly or 25 mg twice wkly. Osteosarcoma Night dose therapy: 12 mg/m2 may be increased to 15 mg/m2 IV or IM at 24 hr after initial infusion. Take leucovorin 15 mg every 6 hr for 10 doses. Psoriasis 10-25 mg/wk by IM or IV.
Contraindications
Hypersensitivity. Serious renal or liver disorders, blood dyscrasias eg, bone marrow hypoplasia, leukopenia, thrombocytopenia or significant anemia. Pregnancy & lactation.
Special Precautions
Metriculous care is required in high dose regimens recommended for osteosarcoma & other neoplastic diseases. Monitor for possible toxic reactions. Infection, peptic ulcer, ulcerative colitis, debility. Perform lab tests eg, hemograms & hematocrit, renal function test & urine analysis, measurements of liver enzyme & X-ray. OC precautions during & at least 3 mth after therapy. Severe, recalcitrant disabling disease unresponsive to other forms of therapy. Discontinue therapy if severe leucopenia, bacterial infections & bone marrow depression occurs.
Adverse Reactions
Ulcerative stomatitis, leukopenia, nausea & abdominal distress; malaise, undue fatigue, chills & fever, dizziness, decreased resistance to infection.
Drug Interactions
Prolonged & elevated serum levels w/ NSAIDs &/or salicylates. Increased serum toxicity w/ phenylbutazone, phenytoin, sulfonamides. Increased incidence or cytotoxic bone marrow depression w/ allopurinol.
