Indications/Uses
Monotherapy for partial seizures w/ & w/o secondary generalization in adults & childn >12 yr. Adjunct for partial seizures w/ & w/o secondary generalization in adults & childn ≥3 yr. Neuropathic pain in adults ≥18 yr.
Dosage/Direction for Use
Epilepsy Adult & childn >12 yr 900-3,600 mg daily in 3 divided doses. May be initiated w/ 300 mg tid on day 1 or w/ 3-day titration: 300 mg once daily on day 1, then 300 mg bid on day 2 & 300 mg tid on day 3. Max: 3,600 mg daily in 3 divided doses. Max interval: 12 hr. Childn 3-12 yr Initially 10-15 mg/kg daily in 3 divided doses for approx 3 days. Effective dose: ≥5 yr 25-35 mg/kg daily in 3 divided doses, 3 to <5 yr 40 mg/kg daily in 3 divided doses. Max interval: 12 hr. Neuropathic pain Adult Initially 300 mg tid or w/ 3-day titration: 300 mg once daily on day 1, then 300 mg bid on day 2 & 300 mg tid on day 3. Max: 3,600 mg daily. Renal impairment w/ CrCl ≥80 mL/min 900-3,600 mg daily, 50-79 mL/min 600-1,800 mg daily, 30-49 mL/min 300-900 mg daily, 15-29 mL/min 150 mg (as 300 mg every other day) to 600 mg daily, <15 mL/min 150 mg (as 300 mg every other day) to 300 mg daily. All doses to be given in 3 divided doses. Patients undergoing haemodialysis Loading dose of 300-400 mg, then 200-300 mg following each 4-hr haemodialysis.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Not effective in absence seizures. Increased seizure frequency w/ sudden discontinuation. Discontinue gradually over a min of 1 wk. Observe for signs of CNS depression eg, somnolence. May impair ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Abdominal & back pain, fatigue, fever, headache, viral infection, asthenia, malaise, facial edema; vasodilation, HTN; constipation, dental abnormalities, diarrhea, dyspepsia, increased appetite, dry mouth or throat, nausea, vomiting, flatulence, anorexia, gingivitis; leukopenia, decreased WBC, purpura; peripheral edema, increased wt; fracture, myalgia, arthralgia; amnesia, ataxia, confusion, abnormal coordination & thinking, depression, dizziness, dysarthria, emotional lability, insomnia, nervousness, nystagmus, somnolence, tremor, twitching, vertigo, hyperkinesia, increased/decreased or absent reflexes, paresthesia, anxiety, hostility; coughing, pharyngitis, rhinitis, pneumonia; abrasion, acne, pruritus, rash; amblyopia, diplopia, abnormal vision; impotence, UTI.
Drug Interactions
Reduced bioavailability w/ antacid. Altered renal excretion w/ cimetidine.
